This privacy policy explains how Five Dock Physiotherapy ("we", "us") collects, uses and protects information when you visit fivedockphysiotherapy.com (the "Site"), contact us, or request an in-home physical therapy visit. Our services are offered only in the United States, and this policy is written for U.S. residents.
Information we collect
Information you give us
- Contact details: name, phone number, email address, state and ZIP code.
- Booking details: preferred dates and times, whether the patient is 65 or older, whether you're booking for someone else, and the general areas you'd like help with (for example, "back pain").
- Messages: anything you choose to write in a form, email or call. Please don't send detailed medical history through the Site; we'll discuss it with you directly.
Information collected automatically
- Device and usage information: browser type, pages visited, referring page, and approximate location derived from your IP address, collected by our hosting provider in server logs.
- Cookies and similar technologies: essential storage to remember your cookie choice and, only if you accept, advertising cookies such as the Meta Pixel. See our cookie policy.
How we use information
- To respond to your request, call you back, schedule visits and match you with a licensed therapist in your state.
- To confirm eligibility for offers such as the 65+ free sessions.
- To send appointment confirmations, reminders and service messages.
- To run, secure and improve the Site.
- With your consent, to measure how well our advertising works.
- To comply with law and protect our patients, staff and business.
Health information
The general area of concern you select on our booking form (for example, "knee and hip pain") may be considered consumer health data under some state laws. We collect it only to provide the service you ask for, and we never sell it or share it with advertisers.
If you become a patient, information in your treatment record is handled according to applicable law, including the Health Insurance Portability and Accountability Act (HIPAA) where it applies, and our Notice of Privacy Practices, which you'll receive at your first visit.
How we share information
We do not sell your personal information. We share it only:
- With your therapist: the licensed physical therapist assigned to your visit.
- With service providers who help us operate, such as website hosting, our form-processing provider (FormSubmit), email and phone services. They may use the information only to provide services to us.
- With advertising partners, only with your consent: if you accept advertising cookies, Meta may receive standard website events (such as a page view or that a request form was sent). We do not send your name, contact details or health concerns to advertisers.
- For legal reasons: to comply with law, respond to lawful requests, or protect rights and safety.
- In a business transfer: if our business is sold or reorganized, subject to this policy.
Text messages and calls
If you give us your phone number and consent, we may call or text you about your request and appointments. Reply STOP to any text to opt out, or HELP for help. Message and data rates may apply. We do not share mobile numbers or text-messaging consent with third parties for their marketing.
Your choices and rights
Depending on where you live, including California, Colorado, Connecticut, Virginia, Texas, Oregon, Washington and other states with privacy laws, you may have the right to:
- Know what personal information we hold about you and get a copy of it.
- Ask us to correct or delete it.
- Opt out of targeted advertising and the "sale" or "sharing" of personal information (we don't sell it).
- Withdraw consent to the collection or sharing of consumer health data.
- Not be treated differently for exercising these rights.
To make a request, email info@fivedockphysiotherapy.com or call (833) 386-9231. We'll verify your identity before acting on it. You may use an authorized agent where your state allows. If we deny your request, you can appeal by replying to our decision.
We honor the Global Privacy Control (GPC) browser signal as a request to opt out of targeted advertising. You can also change your choice at any time with the cookie settings link.
How long we keep information
We keep booking requests and messages only as long as needed to respond and for our legitimate business records, and treatment records for as long as the law requires.
Security
We use reasonable administrative, technical and physical safeguards. The Site uses encrypted HTTPS connections. No method of transmission or storage is completely secure.
Children
The Site is not directed to children under 13, and we don't knowingly collect their information online. A parent or guardian must request any visit for a minor.
Changes to this policy
We may update this policy. We'll change the "last updated" date above and, for significant changes, give additional notice on the Site.
